Two Options: Body or Ashes
Your first decision shapes everything else — the timeline, the cost, and the paperwork.
Shipping ashes is simpler and cheaper. The body is cremated at one of Poland's approximately 80 crematoria, the ashes are placed in a sealed transport urn, and they're shipped via international courier or carried on a flight. Total cost to the US: roughly $2,300-$3,100.
Full body repatriation requires embalming (mandatory for international transport), a specialized metal-lined casket, and air cargo booking. Expect $3,700-$5,500 depending on the destination city. Warsaw to New York runs $3,700-$4,700; Warsaw to Los Angeles can reach $5,500 due to long-haul air freight surcharges.
Required Permits and Documents
The Sanepid Export Permit
The State District Sanitary Inspector (Sanepid — Stacja Sanitarno-Epidemiologiczna) must issue an export permit before any remains can leave Poland. You'll need:
- The registered death certificate (akt zgonu)
- The medical death card (karta zgonu) showing the cause of death
- A sworn Polish translation of any foreign documents if applicable
The Sanepid office verifies there are no public health restrictions (certain infectious diseases can block or delay transport) and issues the permit.
Apostille and Consular Documents
- US citizens: The US Embassy in Warsaw issues a Consular Mortuary Certificate (approximately $60, though free of charge for US citizens in some circumstances). You'll also need the death certificate with an apostille for use in the US.
- UK citizens: Contact the British Embassy in Warsaw. The Foreign, Commonwealth and Development Office provides guidance on transporting remains back to the UK.
The death certificate apostille (authenticating it for international use) costs 60-170 PLN depending on the issuing body.

Exhumation Rules
If the deceased was already buried in Poland and the family later decides to repatriate, exhumation is restricted to the period between October 16 and April 15 (cooler months). There's a mandatory waiting period of at least one year after burial, or two years if the death was caused by an infectious disease.
Exhumation requires a separate Sanepid permit and cemetery manager approval.
The ZUS Funeral Grant
Poland's state funeral grant reimburses up to 7,000 PLN (raised from 4,000 PLN in January 2026). Close family members receive the full flat amount regardless of actual costs. Non-family members are reimbursed for documented expenses up to the 7,000 PLN cap. File Form Z-12 with ZUS within 12 months of the death.
This grant can offset a significant portion of repatriation costs, but ZUS only covers direct funeral and transport expenses — not headstones, flowers, or post-funeral receptions.
