Nevada Lockbox: How to Register Your Advance Directive with the Secretary of State

A signed advance directive locked in a safety deposit box is useless at 2 AM on a Saturday when you are rushed to the ER. Your family cannot access the bank, your agent does not have a copy on their phone, and the attending physician has no way to verify your wishes.

What the Lockbox Does

The Lockbox is a secure digital vault. Once your advance directive is registered, any hospital or healthcare provider in Nevada can retrieve your documents electronically at any hour using your unique registration number and password. No phone calls to family members. No searching through filing cabinets. No delays.

After registration, you receive a wallet card displaying your registration number and password. Carry it with your ID. When you arrive at a hospital — conscious or not — staff can use the card to pull up your directive immediately.

How to Register Step by Step

1. Complete your advance directive first. The Lockbox stores documents; it does not create them. Your directive must be fully executed (signed and either notarized or witnessed) before you submit it.

2. Fill out the Advance Directive Registration Agreement. This is a separate form from the directive itself. It authorizes the Secretary of State to store and provide access to your document.

3. Submit both documents to the Lockbox Division. You can mail or upload the signed agreement and a copy of your executed directive. There is no fee — the service is completely free.

4. Wait for processing. The Lockbox Division typically processes submissions within 7 to 14 business days. You will receive your wallet card by mail once processing is complete.

5. Distribute your registration information. Give your registration number and password to your designated healthcare agent, your primary care physician, and any family members you want to have access.

Updating or Replacing Your Documents

Life changes — you may want to update your agent designation, change treatment preferences, or revoke the directive entirely. To modify what is stored in the Lockbox, you must file an Authorization to Change Form with the Lockbox Division.

This is important: simply executing a new directive does not automatically update the Lockbox. If you revoke or replace your directive but do not file the change form, the Lockbox will continue serving the old version to healthcare providers. That creates exactly the kind of ambiguity you are trying to avoid.

Why the Lockbox Matters More Than You Think

The gap between having a directive and having it honored comes down to accessibility. A physician in a crisis situation will act on what they can verify, not what a family member claims exists. The Lockbox eliminates the verification delay entirely.

Consider this scenario: you are visiting family in Reno but your primary care physician is in Las Vegas. You are admitted to a Reno hospital after a fall.
